  • Peripheral Artery Disease

  • Neuropathy

  • Obesity

• Support from certified risk adjustment coders to ensure appropriate documentation

• Extensive medical records review to assist in:

  • the identification of important health conditions

  • coordination of care among providers

  • support for accurate capture and recapture of HCC conditions

• Preventative Care Screenings including:

  • Pain Assessment

  • Fall risk assessment

  • Depression assessment

  • Substance use assessment

  • Medication review for polypharmacy

  • Vaccine history & recommendations

  • Cognitive assessment

• Coordination with primary providers to ensure cohesive transitions
